“what is good enough? 75%, 90%, 99.9%?
If 99.9% was good enough, then 301 entries in the Oxford English Dictionary would be mis-spelled, 37200 passengers would not arrive safetly in the Changi Airport, 3013 defective mobile phones will be shipped daily.
If 99.9% was good enough, then 32 babies would be discharged to the wrong parents, 775 patients would be given the wrong treatment every month, 4158 polyclinic patients would be mis-diagnosed.
99.9% just isnt good enough.
good is not good when we have to do better.”
